WELCOME TO THE SONOMA COUNTY INDIAN HEALTH PROJECT!

The Sonoma County Indian Health Project, Inc. ("SCIHP") was established in 1971 by a group of visionary leaders from the Indian communities of Sonoma County. Their goals were to have a health center that would afford easy access to high quality care for all Indians of Sonoma County, and to provide such services in a sensitive manner to the culture and traditions of the local Indian people.

Through a special relationship with the California Rural Indian Health Board, SCIHP is a subcontractor with the federal government. In effect, this establishes SCIHP as a tribal organization under provisions of Public Law 93-638 (Indian Self-Determination & Education Act), and as such, performs the functions of the Indian Health Service throughout our assigned service area. This relationship carries certain responsibilities to the tribal governments which comprise our contractual relationship:

  • Cloverdale Band of Pomo Indians of Cloverdale Rancheria
  • Dry Creek Rancheria of Pomo Indians of Dry Creek Rancheria
  • Kashia Band of Pomo Indians of the Stewart_s Point Rancheria
  • Lytton Band of Pomo Indians of Lytton Rancheria
  • Manchester Band of Pomo Indians of the Manchester-Point Arena Rancheria

 

SITE(S)

The main site is the Sonoma County Indian Health Clinic located in Santa Rosa, CA. Currently, a satellite clinic is being operated by SCIHP at Manchester-Point Arena in Point Arena, CA, providing medical, outreach, and environmental health services.

SERVICES

SCIHP is licensed by the State of California as a Community Health Center. We are currently providing medical, dental, nutritional, and behavioral health services. Supportive services include full-service pharmacy, health education, and community health/outreach services.

 

Medical - Diagnosis & treatment for acute & chronic conditions, obstetrical care; general pediatric care; hospital inpatient care; routine physical examinations for general health maintenance & employment requirements; referrals for specialist care; preventive health services; foot care in our Foot and Ankle Clinic provided by a fully qualified Podiatrist.

Service providers: 3 full-time physicians, 1 part-time consulting physician, 1 Family Nurse Practitioner (main clinic), 1 Certified Physician_s Assistant (satellite clinic), 1 Podiatrist, 1 Clinic Nurse Manager (Registered Nurse, main clinic), 1 Clinic Manager (Registered Nurse, satellite clinic), 2 Licensed Vocational Nurses and 1 Certified Medical Assistant

Dental - A broad range of general family dentistry: an ongoing program of dental health promotion, health maintenance, restorative services and preventive care.

Service providers: 2 full-time dentists, 1 part-time dentists, 1 part-time orthodontist, 4 Registered Dental Assistants

Behavioral HealthServices -Mental health, clinic social work, and substance abuse treatment and referral activities.

Service providers: 1 licensed Marriage, Family and Child Counselor (MFCC), 1 licensed Clinical Psychologist, 2 Licensed Clinical Social Workers (LCSW), and an Alcoholism/Substance Abuse Counselor.

Nutritional Services - A WIC Program; two congregate melas programs for seniors; a special food supplement program for seniors at Stewart_s Point (Kashia), Dry Creek, Cloverdale, and Lytton Rancherias; special prgrams for diabetics, infants, and expectant mother; and an ongoing program of nutritional counseling.

Service providers: 2 Registered Dieticians (RD), 1 each WIC Program Manager & Assistanct Program Manager, and 2 Senior Nutrition Site Managers

Community Health Services - Outreach; hospital and nursing home visitations; home health care services; immunization follow up visits; special care for the chronically ill; assistance with entitlement programs; communicable disease follow up; community injury control; patient education; environmental health services; and limited transportation services for the elderly, disabled, infants, and young children

Service providers: 1 Public Health Nurse and 6 Community Health Representatives (CHR)

 

Pharmacy Services - SCIHP's Pharmacy is licensed by the State of California as a full-service retail pharmacy and is able to provide the highest quality of pharmaceutical products available on the market.

Service providers: 1 Registered Pharmacist and 1 Pharmacy Clerk

Contract Health Services - A restricted Federal program of managed care. In the five general medical priority levels, SCIHP is authorized to provide for the first three levels only, covering emergency/acutely urgent care, acute primary and preventive care, and chronic primary and secondary care. Note: most Level II services and many in Level III are available directly from SCIHP.
