The Adolescent Alcohol and Drug Involvement Scale (AADIS) was developed at the University of Wisconsin-Madison by Dr. Paul Moberg and can be downloaded from his website. There is no charge to download and use the AADIS. The Addiction Severity Index (ASI) was developed by the Treatment Research Institute and can be downloaded from their website. There is no charge to download and use the ASI. The CAGE Questionnaire was developed by John Ewing and was published in 1984 in the Journal of the American Medical Association (JAMA).
